OBESITY TREATMENT AND BARIATRIC SURGERY CENTER

Obesity Treatment and Bariatric Surgery Center at our Private Çorlu Vatan Hospital was founded to provide a solution to one of the most important health problems of today’s society – obesity. We have three experienced doctors practicing at our center, specialists with years of experience in the field of bariatric surgery. WHAT IS OBESITY?

Today, obesity represents one of the most crucial health issues in both developed and developing countries. Typically, obesity involves the accumulation of an excessive amount of fat tissue in the body. This deposition of fat tissue can be subcutaneous, in the abdominal cavity, around the internal organs. According to the body mass index (BMI) classification, a BMI over 25 indicates overweight, and a BMI over 30 indicates obesity. Obesity can adversely impact the patient’s quality of life, social life, and mental well-being, as well as lead to serious illnesses.

In overweight patients, a significant increase in the risk of developing cardiovascular diseases, type 2 diabetes, fatty liver disease, joint disorders, and an elevated risk of developing cancer is observed. In this regard, it is known that obesity is not only an issue related to the patient’s aesthetic appearance but is a condition with serious consequences for health, and is one of the main factors that shorten life.

When deciding on surgical intervention for patients with obesity and metabolic disorders, it is done based on the body mass index (BMI). In order to decide for surgical intervention in patients with obesity without accompanying diseases, the body mass index should be over 40. For those patients with accompanying diseases such as type 2 diabetes, the body mass index should be over 25. Upon deciding on surgical intervention, patients are thoroughly informed about the necessary procedure, expected benefits, the postoperative period, and the diet they will need to adjust during this period.

NON-SURGICAL PROCEDURES

The application of Botulinum Toxin (Botox) to the stomach is a weight-loss procedure involving the injection of botulinum toxin into specific parts of the stomach through an endoscopic method. Botox decelerates digestion in the stomach, extending the period of satiety by slowing down the digestion of food.

The procedure typically takes approximately 15-20 minutes and is performed through an endoscopic method, under mild sedation, without pain and nausea. The main advantage of this non-surgical procedure is the immediate return of the patient to their normal routine, with the absence of incisions, stitches, or the need for postoperative dressing. So far, the utilization of botulinum toxin has no known dangerous side effects.

This intervention aims to achieve a weight loss of approximately 10-15% of the total body weight within a period of 3-6 months. The extent of weight loss may vary individually, influenced by factors such as age, metabolism speed, and physical activity levels.

The Intragastric Balloon, also known as the “Gastric Balloon,” is a medical device designed to help patients lose weight without surgery. The placement of the balloon into the stomach is done through the oral cavity, using an endoscopic method. The gastric balloon consists of a soft, expandable balloon, an inserted tube, and a filling system. The purpose of placing the gastric balloon is to partially fill the stomach, assisting in creating a consistent feeling of fullness even with minimal food consumption. This is a reversible and straightforward technique that does not require incisions, making it one of the interventional diets.

Weight loss through the placement of the Intragastric Balloon varies depending on the patient’s current excess weight, metabolism rate, gender, age, metabolic and hormonal conditions, commitment to the diet, and physical activity. The expected average weight loss is around 20-30% of the current weight. Commitment to the diet and increasing physical activity will further contribute to achieving higher results.

SURGICAL PROCEDURES

Sleeve Gastrectomy, also known as “Gastric Tube Surgery”, is the most commonly used surgical method among obesity surgery methods. It is a safe and highly effective surgical procedure for weight loss, allowing patients with issues such as obesity to achieve permanent weight loss. The “Sleeve Gastrectomy” intervention is a laparoscopic surgical procedure, meaning that through minimal incisions made in the upper part of the abdomen, approximately 80% of the stomach is removed, while the remaining part takes on the shape of a tube. This intervention does not involve any intervention in the intestines.

The average expected weight loss within one year after the procedure ranges from 40% to 70%. One of the main advantages of this method is that no changes are made in the small intestine, the foods follow the normal digestive tract, and thus it is unlikely to encounter vitamin and mineral deficiencies in the postoperative period.

Gastric Bypass is a combined method involving a two-step laparoscopic surgery. The “Gastric Bypass” procedure involves technically reducing the size of the stomach by minimizing it to a small pouch through the use of staples. Staples are placed in the upper part of the stomach to create a small pouch isolated from the remaining part of the stomach. This pouch empties directly into a distal section of the small intestine connected to it. The proximal section, following the natural course of the gastrointestinal tract, is then connected to the lower section of the intestine after isolating the portion of the stomach.

Unlike procedures such as sleeve gastrectomy, the remaining part of the stomach is not removed, but no food is accessible in the remaining part of the stomach. Gastric juices and enzymes continue to be produced by the remaining portion of the stomach to assist in digestion and nutrient absorption. Thus, consumed foods bypass most of the stomach and the initial part of the small intestine. As a result, patients experience a sensation of fullness even with a small amount of food, while a reduced amount of food is absorbed by the small intestine.

Patients who undergo the “Gastric Bypass” procedure typically lose two-thirds (2/3) of their weight within 2 years. This weight loss yields positive outcomes in the treatment of patients with type 2 diabetes, providing a permanent solution to numerous other conditions caused by excess weight.

Metabolic surgery can provide medication-free control of uncontrolled type 2 diabetes. In cases where patients present with similar complaints are treated in a multidisciplinary manner at our clinic. Initially, a detailed physical examination and assessments are carried out by our Internal Medicine specialists to make decisions regarding the surgical treatment of type 2 diabetes.

For cases with elevated blood glucose levels that cannot be controlled through diet and medication, the most suitable surgical intervention is chosen among gastric bypass, transit bipartition, and ileum interposition. After the surgical intervention, our patients are placed under the direct supervision of a team of specialists, including a dietitian, internal medicine specialist, and general surgery specialist.

FREQUENTLY ASKED QUESTIONS

The primary causes leading to obesity are improper eating habits, an inactive lifestyle, and certain endocrine disorders such as hypothyroidism and Cushing’s syndrome, among others. Obesity can negatively impact the individual’s quality of life, social interactions, and mental well-being, while also leading to serious medical conditions. In overweight patients, there is a significant increase in risk of developing cardiovascular diseases, type 2 diabetes, non-alcoholic fatty liver disease (hepatic steatosis), bone and joint disorders, and an increased risk of developing cancer.

In this regard, it is known that obesity is not merely an issue related to the patient’s aesthetic appearance but is a medical condition with serious consequences on one’s health, acting as a key factor in shortening human lifespan.

Non-surgical methods of treating morbid obesity can be classified as following a strict diet, intragastric balloon insertion and gastric botox insertion.

The most common surgical interventions performed in the field of bariatric and metabolic surgery are “Sleeve Gastrectomy”, “Gastric Bypass” and “Transit Bipartition”. The appropriate surgical approach for the individual patient is determined after evaluating multiple factors such as physical condition, comorbid (co-morbid) diseases and patient lifestyle, etc.

Not every overweight patient is suitable for surgery. The decisive factor here is the body mass index(BMI) and the presence of additional conditions such as type 2 diabetes.

Patients undergo physical examinations by specialists from various fields and detailed laboratory tests before a final decision is made for surgical intervention. Preoperative preparation includes consultations with a cardiologist, internal medicine specialist, pulmonologist, psychiatrist, nutrition and dietetics specialist, along with detailed blood tests.

After these examinations and tests, if it is assessed that the patient meets the necessary conditions for surgical intervention, a consultation with a specialist in anaesthesiology and intensive care specialist is also carried out. In cases of no identified medical obstacles, the surgical intervention is performed. However, in cases of any identified medical obstacles during examinations and tests, necessary treatments are carried out to eliminate these preconditions before considering the surgical intervention. Once the patient’s condition stabilizes, the possibility of surgical intervention is reassessed.

The duration of the surgery varies according to the operation performed. Sleeve gastrectomy takes about 1-2 hours, gastric bypass about 2-3 hours, and bipartition about 3 hours. The average hospital stay is 3-4 days. The patient resumes daily life immediately and returns to work in 10-15 days.

Surgical interventions in the field of bariatric surgery are performed under general anesthesia. All operations are performed with utmost care and precision by our experienced specialists in the field of bariatric surgery.

Each individual stage of the surgical intervention requires maximum attention. Determining the right surgical approach for the patient is as crucial as the surgery itself. The materials and devices we use during surgical interventions are of the highest quality and meet global quality standards.

The primary concerns during surgical interventions are the occurrence of respiratory problems, thromboembolism, hemorrhage, and the formation of inguinal hernias. For patients undergoing the Sleeve Gastrectomy procedure, the primary concern is the formation of staple line leaks (the area of suture). The world leading statistics in the medical literature indicate that this occurs in 2-4% of cases.

In the postoperative process, patients should prioritize adequate fluid intake, adherence to a prescribed diet, and regular follow-ups with both a nutritionist and dietitian. However, the crucial aspect of this process involves deciding a permanent lifestyle change in terms of eating habits and daily activities. The observed weight gain in the subsequent years after the surgical intervention is mostly due to irrational eating.

Our 54-year-old patient, Mrs. Aysel, who had complaints about her weight, applied to our hospital and to the Obesity Surgery physician, Dr. Okay Pırtı after starting to experience various health problems. Upon the planned visit to our hospital, Dr. Mustafa Önder DOLAP, our specialist in General and Bariatric Surgery, took charge of his case. After the initial physical examination and consultation with the specialist, a series of physical examinations and tests were carried out to determine the patient’s current condition.

On the same day, the patient underwent a detailed examination, including physical examinations by specialists from various branches such as Internal Medicine, Cardiology, Psychiatry, Nutrition, and Dietetics. Numerous blood and radiological tests, fibrogastroscopy, abdominal ultrasound, and more were performed. Based on the results of these examinations, it was determined that there were no medical obstacles to the planned surgical intervention.

Following the analysis of the detailed examinations and studies, Dr. Mustafa Önder DOLAP recommended the “Transit Bipartition” intervention.
The surgical procedure was successfully performed the next day, and the patient was then admitted to our surgical unit for postoperative follow-up.

After a one-week hospitalization and follow-up, the patient was discharged from the hospital with a smile and in good overall health.

Four months after the initial control examination, our patient returned to the hospital for a routine check-up. Measurements revealed that the patient had lost a total of 53 kg in 5 months. Additionally, the fat ratio decreased from 47% to 34%, and his waist circumference reduced from 190 cm to 130 cm.

We are extremely happy to see the positive results and the transformation in our patient, who stated that he is very happy with his new lifestyle.
