OBESITY CENTER AND BARIATRIC SURGERY
The Center for Obesity Treatment and Bariatric Surgery at Çorlu Vatan Hospital was established to provide a solution to one of the most important health problems in today’s society: obesity.
Three experienced doctors, specialists with many years of experience in the field of bariatric surgery, practice at our center. From its foundation until today, this center has successfully cured many patients, and with each passing day their number continues to grow. It is for this reason that our center has reached a high level of experience in the field of bariatric and metabolic surgery. It offers surgical and non-surgical methods for the treatment of obesity, according to the health condition, needs and expectations of each patient.
WHAT IS OBESITY?
Today, obesity is one of the most important health problems in developed and developing countries. Obesity is usually the accumulation of excessive amounts of fat in the body. This deposition of adipose tissue can be subcutaneous, in the abdominal cavity, around the internal organs. According to the classification according to the body mass index (BMI), for an index over 25 we are talking about being overweight, and for an index over 30 we are talking about obesity.
Obesity can adversely affect the quality of life of the sufferer, social life and mental state of the person, as well as cause serious illness. In overweight patients there is a dramatic increase in the risk of cardiovascular diseases, type 2 diabetes, fatty liver, joint diseases, as well as an increased risk of developing cancer. In this regard, it is known that obesity is not just a problem related to the aesthetic vision of the patient, but a disease that has serious consequences for health and is one of the main factors that shorten life.
When deciding on surgery, in patients with obesity and metabolic disorders, proceed according to body mass index (BMI). In order to decide on surgery in obese patients without concomitant diseases, the body mass index should be above 40. And for those patients who also have concomitant diseases such as type 2 diabetes, the body mass index should be above 25.
After deciding on surgery, patients are informed in advance about the necessary intervention, the expected benefits of it, the postoperative period and the diet to which they will have to adhere during this period.
NON-SURGICAL PROCEDURES
The application of Botulinum Toxin (Botox) to the stomach is a weight-losing procedure that is based on injecting botulinum toxin into the particular parts of stomach through an endoscopic method. Botox helps to slow digestion in the stomach; this slows down the digestion of food and thus the satiety period becomes longer.
The procedure takes about 15-20 minutes. The placement of gastric Botox is performed by endoscopic method without pain and nausea, with light sedation. The main advantage of the procedure is that it is not a surgical procedure so the patient immediately returns to the normal routine. As there are no incisions and stitches, no postoperative dressing is required. To date, the use of botulinum toxin has no known dangerous side effects.
Through this procedure, patients are expected to lose about 10-15% of their total weight in a period of 3-6 months. The amount of weight lost may vary for each person depending upon age, metabolism speed and physical activity.
Known as Intra-Gastric Balloon and Stomach Balloon, is a medical product that aims to help patients lose weight without surgery. The introduction of a balloon into the stomach is done through the oral cavity, by endoscopic method. The purpose of placing a stomach balloon is to partially fill the stomach, which helps to create a constant feeling of satiety, even with a minimum amount of food. This is a reversible and simple technique that does not require incisions, so it is considered one of the interventional diets.
The weight loss by using an Intra-Gastric balloon varies for each person depending upon the excessive weight, metabolism speed, gender, age, metabolic and hormonal diseases, adaptation of the person to diet and physical activities. The average expected weight loss is about 20-30% of your current weight. Adherence to diet and increased physical activity will further help to achieve higher results.
SURGICAL PROCEDURES
Sleeve Gastrectomy is the most commonly used surgical method among surgical methods for obesity surgery. This is a safe and highly efficient surgical method for weight loss that allows patients with problems such as obesity to lose weight permanently. Sleeve Gastrectomy is conducted by using laparoscopic surgical method, which means that the procedure will be carried out through small incisions in the upper abdomen area. The procedure involves the removal of approximately 80 % of the stomach, while the remaining part of the stomach takes the shape of a tube. No intestinal intervention is performed during this intervention.
After the Sleeve Gastrectomy surgery, patients are expected to lose 40% to 70% weight within a year on average. As no changes are made in the small intestine and the foods follow the normal digestive tract and so the possibility of lack of vitamins and minerals is low in the postoperative period.
Gastric bypass is a combined method consisting of two-stage laparoscopic surgery. The first step is technical reduction in the size of the stomach by minimizing the stomach to a small sac with staples. With it, a small cavity isolated from the rest of the stomach is formed in the upper part of the stomach. The proximal part, which follows the natural course of the gastrointestinal tract, after the isolated part of the stomach, connects with the lower part of the intestine. The rest of the stomach is not removed, as in the case of a Sleeve Gastrectomy, but there is no access to food in the rest of the stomach. Gastric juices and enzymes continue to be produced by the rest of the stomach to aid digestion and absorption of nutrients. In this way, the food consumed surrounds most of the stomach and the first part of the small intestine. Thus, the patient gets a feeling of satiety, even when consuming a small amount of food, and at the same time less food is absorbed by the small intestine.
Patients who have undergone Gastric Bypass surgery usually lose two-thirds of their weight within 2 years. As a result of this weight loss, positive results have been achieved in the treatment of patients with type 2 diabetes, as well as a lasting solution to many other diseases caused by overweight.
The purpose of metabolic surgery is to ensure the control of the non-controlled type II diabetes without administering any drugs. Patients who come to our clinic with such complaints are approached multidisciplinary. Initially, in order to decide on the surgical treatment of type 2 diabetes, a detailed physical examination and research is performed by our specialists in internal medicine.
In cases with high blood glucose levels that cannot be controlled with diet and medication, the most appropriate surgical intervention is chosen between Gastric Bypass, Transit Bipartition and Ileal Interposition. After the operation patients are under the direct supervision of our physicians of our Obesity and Metabolic Surgery Clinic (Nutritionist, Internal Diseases specialist, General Surgery specialist).
FREQUENTLY ASKED QUESTİONS
Obesity, known also as overweight, means having excess body weight. Body mass index is taken into account when classifying obesity. BMI is calculated as follows: according to the square of the weight/height ratios (in meters) and the resulting value, the classification is made between extreme thinness and morbid obesity.
The causes of obesity are primarily bad eating habits, sedentary life, and some endocrine diseases such as hypothyroidism and Cushing.
Non-surgical methods in obesity include diet, Gastric Balloon and Botox injections in the stomach.
The most commonly used surgical methods in obesity and metabolic surgery are sleeve gastrectomy, gastric bypass, and bipartition surgeries. The surgical method is determined according to the patient’s additional diseases and lifestyle.
Not all obese patients are suitable for obesity or endocrine surgery. Here, the body mass index and the presence of additional diseases such as diabetes are decisive.
Patients undergo very detailed examinations and laboratory tests prior to the operation. After cardiology, internal medicine, chest diseases, psychiatry, and nutritionist examinations, as well as detailed blood tests the anesthesia visit is realized and then necessary treatment is applied if necessary and then the patient become ready for the surgery.
The duration of the surgery varies according to the operation performed. Sleeve gastrectomy takes about 1-2 hours, gastric bypass about 2-3 hours, and bipartition about 3 hours. The average hospital stay is 3-4 days. The patient resumes daily life immediately and returns to work in 10-15 days.
The surgery is performed with general anesthesia. We perform the surgery with our experienced surgical team.
Every stage of the surgery is carried out with the utmost care. Therefore, determining the best surgery for the patient is just as important as the surgery itself. We use materials and devices that are globally accepted by international standards in our surgeries.
Lung complications, thromboembolism, bleeding, leaks, and internal hernias are the most concerning complications during surgery. Especially in sleeve gastrectomy patients, the most common fear is relative of leaks fistula. It is reported in the literature at a rate of 2-4%.
Adequate fluid and calorie consumption and regular nutritionist follow-up are extremely important after the surgery. The most important issue, however, is a permanent change in lifestyle in terms of nutrition and daily activities. The weight gain observed over the years is mostly due to this reason.

M.D. Mustafa Önder DOLAP
Surgeon M.D. Mustafa Önder Dolap, a General Surgery Specialist and also the assistant head physician of our Hospital, was graduated from Istanbul Faculty of Medicine in 1996. He completed his specialization education at Istanbul Haseki Training and Research Hospital. In 2013, he completed his Minimal Invasive Surgery education at Strasbourg University and received Obesity and Metabolic Diseases Surgery education at Cerrahpaşa Faculty of Medicine in 2015.

M.D. Ferudun BIKMAZ
Surgeon M.D. Ferudun Bıkmaz, a General Surgery Specialist at our Hospital, was graduated from Istanbul Faculty of Medicine in 1987. He completed his specialization education at Beyoğlu State Hospital in 1993. He had worked at various hospitals of the Ministry of Health until 2015 and has been working at Çorlu Vatan Hospital since 2015.

M.D. Okay PIRTI
Surgeon M.D. Okay Pırtı, a General Surgery Specialist at our Hospital, received education at Ankara University Faculty of Medicine between 2000 and 2006. Subsequently, he received his general surgery specialization education at Ankara Training and Research Hospital between 2006 and 2012. He had worked at three hospitals until 2018 and he has been working at Çorlu Vatan Hospital since 2018.

Our patient came to our Hospital from Sweden for Sleeve Gastrectomy Operation
Mr. Stoyan Turnov, who is living in the Kingdom of Sweden for many years, contacted our International Patients Department on the recommendation of a friend that undergone to Sleeve Gastrectomy Surgery in our Hospital. He said us about his desire to find a lasting solution to his overweight problems. After the detailed preparation for his visit, our patient was met by our colleagues from Istanbul Airport and was transported to our Hospital in Çorlu.
Mr. Turnov’s case was taken over by M.D. Mustafa Önder DOLAP – Specialist in General and Bariatric Surgery and Deputy Chief Physician of our Hospital. During the consultation with Dr. Mustafa Önder DOLAP, the patient was provided with detailed information about the most appropriate method of treatment in his case – the intervention “Sleeve Gastrectomy”, which was determined based on the Body Mass Index and health condition of Mr. Turnov.

Our general surgeon Ferudun Bıkmaz held a consultation in Burgas city of Bulgaria, there he met the patients and discussed about their disturbs and possibly treatments. Mrs. Safije, 62-year-old, came to our Obesity and Bariatric Surgery Center from Macedonia.

After a two-hour laparoscopic operation, 80% of the patient’s stomach was removed and next she was placed back in her room to rest. The four-day hospitalization was required for the postoperative follow-up of Mrs. Safije Idrizi, in this way she was under the direct supervision of our and nutrition and dietetics specialist, Çiğdem Kıral and also general surgeon.
On the day of the patient’s discharge, it was measured that she lost 8 kilograms during her hospital stay. Our patient Atanas Ivanov Bozhinov, from Varna Province of Bulgaria, contacted our department for international patient care after coming across our publications on social networks. During the conversations, he told us that he has problems with being overweight, has complaints such as high blood sugar and has difficulty performing his daily activities.
After the talks with Mr. Bozhinov, on the basis of the information shared by him, a preliminary consultation was made with our specialist in General and Bariatric Surgery M.D. Ferudun BIKMAZ and the patient was invited to our hospital for detailed examination. Upon his arrival at our hospital, Mr. Bozhinov’s case was taken over by M.D. Ferudun BIKMAZ. After a series of physical examinations by specialists from various medical departments and detailed examinations, based on the current condition of the patient, the intervention “Sleeve Gastrectomy” was recommended. During the ultrasound, which is part of the routine preoperative preparation, it was
found that Mr. Bozhinov’s gallbladder was infected due to the presence of gallstones, which mechanically block the bile duct. Based on the data obtained from the preoperative preparation, a decision was made to perform surgery by laparoscopic method, aimed at solving both problems of the patient.
The intervention was performed by laparoscopic method, first removing the gallbladder and then 80% of the patient’s stomach.

Our 54-year-old patient, Ms. Aysel, who had complaints about her weight, has applied to our hospital and to the Obesity Surgery physician, M.D. Okay Pırtı after starting to experience various health problems.
